You should print the form they sent you out from your computer, fill it manually and send it now to them.
Send them a copy of your Driving Licence because they have complained to our office already that the passport you sent to them expired since 2018 and so therefore it is not acceptable. So you must send to them a copy of your Driving Licence immediately. They do not need any notarization from you for now. Just do exactly what they have directed you to do.
You must fill the form correctly and return it back to the bank immediately and without any delay. Thank you.
